Hospitalists in Academia: Lessons Learned from Adult Medicine
Scott Flanders, MD

Dr. Flanders provided an overview of the evolution of the discipline of hospitalist
medicine since Wachter and Goldmann’s landmark article in 1996. He discussed
the literature demonstrating the successes of hospitalists in internal medicine,
and the relevance of these lessons to pediatrics. He described future clinical,
research, and educational challenges that will need to be met for the field
to move forward.

Dr. Ottolini led a workshop discussing variability in the definition of hospitalists
in pediatrics across the country. Issues covered included remuneration, settings
of care, work hours, and organization of services. The emphasis was on the
work hospitalists do when on service as hospitalists, but the session also
touched on hospitalists’ other roles’ in academic centers.
Hospitalist as Consultant (include Billing)

Dr. Chiang led session describing how hospitalist leaders can advocate for
themselves in negotiations with payors and hospitals. He discussed issues in
billing, primary responsibility for patients, and the role of the hospitalist
as consultant.
Role of Hospitalist Director (inc. Building Networks in your Community)
Pat Lye, MD, MS

Medical Director - Special Needs Program (CHW)
Dr. Lye led a discussion of the role of the hospitalist as director of the
inpatient service / service chief. She will discuss both internal hospital
issues, and the role of the hospitalist director in establishing relationships
in the larger medical community.
Coordination of Care – The Medically Complex Child
Raj Srivastava, MD, MPH

Dr. Srivastava led a discussion of hospitalists’ roles in caring for
children with chronic illness and complex needs. This session addressed both
in-hospital issues in the care of these children, as well as the role of the
hospitalist in helping to seamlessly integrate inpatient and outpatient care.

Building an Academic Career - (Promotion tracks, mentoring, leadership)

Dr. Ludwig led a discussion of feasible academic career paths for hospitalists
in academic centers. He drew on his extensive experience in emergency medicine,
and drew parallels between the options available for hospitalists and emergency
medicine physicians, including teacher-clinician and clinical investigator
paths. He discussed the importance of mentoring, and opportunities for leadership.

Drs. Landrigan (chair of the newly formed PRIS hospitalist research network)
and Wasserman (chair of the highly successful, well-established PROS research
network) led a discussion of the challenges and opportunities for collaborative
research in pediatric hospitalist settings.
Quality/Safety Issues

Dr. Muret-Wagstaff used her experience with Six Sigma training and as a Baldridge
examiner to lead a discussion of the application of quality improvement methodologies
to the enhancement of inpatient pediatric care.
Publications – What’s Needed: Getting the Message Out
Lisa Zaoutis, MD

Dr. Zaoutis led a discussion of the various forums through which the voice
of pediatric hospitalist medicine can be expressed. Advancing the identity
of inpatient pediatrics through textbooks, peer-reviewed journals, newsletters,
case studies, etc. was explored.

Dr. Landrigan discussed the recent emergence of pediatric hospitalist fellowship
programs at Children's Hospital Boston and Children’s National this past
year, and the expected emergence of several more in the near future. Implications
for future training and credentialing of hospitalists were also discussed.
CME for the Hospitalist

Dr. Percelay led a discussion of CME for hospitalists – opportunities
currently available, development of competencies, and possibilities for the
future.
